About the Role

The newly created position will contribute to the design and implementation of the “Village Model” which is Council’s ongoing response to supporting older people in the City of Port Phillip to age well. Ensuring that community has a responsive, tailored and individualised central point of contact into the Council that creates links into services, supports and programming within the City, underpinned by a social prescription model.

We currently have 1 permanent part time (15 hours per week) role available 

Duties:

  • Support older people to connect into the services and supports they need to age well
  • Providing referral pathways to community activities that have an early intervention and health prevention focus as well as support access formal support such as My Aged Care.
  • Identify emerging community needs, community expectations and aspirations related to active and healthy ageing and work to address these gaps through partnerships and networks.
  • Lead the development of resources and information that supports access to services, programming and social connection opportunities.

Skills and Experience 

  • Hold a tertiary and/or post graduate qualification in social services, health sciences and/or other relevant field.
  • Demonstrated significant experience within the community aged care or community health sector, with a comprehensive understanding of State and Federal Governments strategic directions and aged care reform agenda.
  • Experience in working one on one with the community to identify needs and link into services and supports that promote ageing well.
  • Demonstrated experience in the development and implementation of programs that meet community needs and or emerging needs and gaps.
  • Demonstrated experience in advocating for community either at an individual or systemic level.
