As our Community Intake and Assessment Officer you will be responsible for Community Service Intake and Assessment of clients entering our services and requiring supports either via My Aged Care and the Commonwealth Home Support Program (CHSP) and Home and Community Care Younger Persons Program (HACCPYP) Programs. In this position you'll be responsible for the assessment and coordination of HACCPYP eligible clients to be linked to appropriate community supports.
You'll be part of the Aged and Disability Services team that undertakes planning, policy, advocacy, partnership and service delivery roles in providing quality home and community support services for older people, and people with a disability and their support and in the development of strategic plans for our older community.
