Aged and Disability Services undertakes planning, policy, advocacy, partnership and service delivery roles in providing quality home and community support services for older people, and people with a disability and their support and in the development of strategic plans for our older community. About the Role:

  • Provide support and assistance for consumers to undertake a range of activities and to facilitate community engagement,
  • Meeting the consumer's need for social contact and/or company in order to participate in community life.
  • Services will include: Social Support Individual, HACC Program for Younger People (PYP) and may also include Relief Delivered Meals Driver / Packer roles as required. Services may be provided within the home, or while accessing community services.
  • Activities may include but are not limited to food shopping, personal shopping, social activities and accompanying to health appointments. This role will
  • Support some consumers with Personal Care, Domestic Assistance and Respite care.
 

Skills, experience and certificates

  • Attained Certificate III Individual Support (formerly known as Cert III HACC) or equivalent.
  • The responsibility for ensuring that all work is undertaken in a safe manner and is not detrimental to the health and safety of consumers and fellow employees.
  • Ability to report where there is a variation in a consumer’s health or Person Centred Plan, or request for increased services.
  • To carry out ongoing safety checks on households and to report any unsafe situations in a consumer’s home.
  • To establish a working relationship with each consumer, and to contribute towards enhancing their self-esteem and independence.
  • Extensive experience in working with challenging consumers and ability to relate to people with a variety of disabilities or age-related disorders.
  • Maintaining professional appearance and attitude in order to market the services of Banyule City Council.
  • Own a safe and reliable car to undertake the Social Support Individual duties.

Mandatory Pre-employment screenings:

  • Driver's Licence
  • Police check
  • Functional Capacity Assessment
  • In addition, willing to obtain a Working with Children's Check
  • Current First Aid / CPR Certificate, or willing to obtain
