THE POSITION

The Care Facilitator works with Consumers/Participants their carers and person responsible to facilitate the planning and implementation of person centred quality services which enable the Consumers/Participants to maintain their dignity, respect and ability to live independently doing the things they choose.

 Reporting to Coordinator Activity and Care Services, you will:

  • Ensure strict maintenance of professional boundaries and confidentiality during all client interactions.
  • Sustain a high level of expertise and skill to craft individualised supports that uphold independence while aligning with client goals like Enablement, Reablement and personal choices.
  • Plan, coordinate, implement, and manage moderately complex Care Plans by:  facilitating client access to an array of services offered by various medical and allied health providers,
  • assigning contractors, generating quotation forms, and overseeing work in compliance with funding guidelines, individual budgets, and the Council’s Procurement Policy.
  • Establish and uphold Service Agreements and Budgets under governmental programs such as the National Disability Insurance Scheme (NDIS), Commonwealth Home Support Program (CHSP), and Home Care Packages (HCP’s).
  • Operate within a defined scope that contribute to developing work methodologies and setting client outcomes within the organisation's clear objectives and financial limitations.
  • Utilise professional judgement and initiative, drawing from specialised knowledge and experience.
  • Address problems by referencing documented techniques, precedents, guidelines, or instructions.
  • Train, supervise and mentor new or temporary staff to aid their learning and development.
  • Prepare and electronically transmit necessary statistical data for entities like My Aged Care (MAC), the Data Exchange (DEX), and Medicare.
  • Exercise due diligence in relation to Work Health and Safety risks, considering the "duty of care" and “dignity of risk” owed to the consumer/participant, while respecting each consumer/participant’s rights to self-determination, independence, and dignity.
  • Work both independently and as part of a team to project and establish workflow priorities for deliverables within the designated responsibility area.

WHO WE ARE LOOKING FOR  

Specifically, you will have:

Essential 

  • Sound working knowledge and understanding of The Aged Care Quality Standards, the NDIS Practice Standards and Quality Indicators.
  • Outstanding proficiency in verbal and written communication, demonstrating the capacity to produce thorough client documentation.
  • Sound computer skills, in particular data entry and word processing, using the Microsoft suite of software
  • Class C driver's licence
  • Ability to meet the requirements for a Criminal Record and Police Check, NDIS Worker Screening Certificate and NSW Working with Children Check

 Desirable

  • Certificate IV in Community Services (Community Work, Aged Care or Disability Work) or other relevant discipline
  • Proven expertise and understanding in budgeting and accounting, supported by relevant experience
  • Credentials and/or applicable experience linked to social work or providing assistance for individuals experiencing cognitive impairment or vulnerability.
  • A proactive individual capable of collaborating with clients and their representatives to develop support plans that align with their preferences, requirements, and financial considerations.
  • Experience working with the Filemaker Pro Client database
  • Current First Aid Certificate
