The Opportunity

The Active Ageing team has an exciting opportunity for a passionate, experienced and suitably qualified Team Leader Active Ageing Services to provide leadership, management and support to Active Ageing team members. Reporting to the Manager Community Wellbeing, you will responsible for delievering key services (Food Servces, Home Maintenance and Modifications, Social Support and Respite) to eligible community members. In addition to this, you will also be responsible for ensuring compliance with relevant funding requirements.

Key Responsibilities

  • Lead and manage the Active Ageing team in delivering high quality services aligned with relevant funding guidelines and based on relevant quantitative and qualitative service data.
  • Lead and coordinate the service planning, monitoring and review of all Active Ageing services.
  • Lead and coordinate referrals for Active Ageing Services within set timeframes, inclusive of referral requirements when needed.
  • Manage all aspects of funding agreements inclusive of reports, acquittals, updates and updated client information.
  • Manage all relevant stakeholder relationships required to deliver Active Ageing services, including the management of contracted providers.

About You

To be considered for this role, you will have the following:

  • Tertiary qualifications in Social Work, Community Services or a related discipline.
  • Understanding of the Aged Care sector and Commonwealth Home Support Program.
  • Strong experience in leading teams to deliver high quality services that meet contractual and legislative requirements.
  • Current Victorian Drivers Licence.
  • Current Working with Children Check.
  • No relevant criminal record found in a Police Check.
